How do I know if I have sleep apnea?

Is Chronic Care Management for every one of my Medicare patients?
No, Chronic Care Management is only for those Medicare patients who have 2 or more chronic conditions.
Will I need a new EHR system for this?
No. You don't. I do all the work on my end using a CCM-specific system. All the notes and summaries either get uploaded to whichever EHR system you use, or you have access to view and download all the records you need.
How secure is the system you use?
I use a top-rated and secured system. In addition, more specific security measures are implemented to ensure patient information is always safe and protected.
Does the patient pay anything?
As with all Medicare programs, the patient has an $8 co-pay, which I go over with them when I review and obtain the consent for the program.
Do I need to pay you any money upfront?
No. You don't pay me anything until you bill and get paid by Medicare for any patients whose care I managed through the CCM program.
If I sign my practice on to your CCM program, will you be considered my employee?
No. I am never your employee. The CCM program guidelines come from the CMS. They direct how it needs to be implemented. I sign a contract with you as an independently contracted business.

There are none. Absolutely none. Caution does need to be applied to ensure patient information is safe and CCM is effectively affecting quality outcomes in the positive.
